I had to set up a label template. Maybe this will help you.
1. Open Word for Mac
2. New Document
3. Mailings
4. Labels
5. Options
6. New Label
7. Set Name [Library Shelf Labels about ¾” x 5”
8. Top margin: 1.375” (it will show 1.37”)
9. Label height 0.6875” (it will show 0.69”)
10. Side margin: 0”
11. Label width: 5”
12. Vertical pitch: 0.6875” (it will show .69”)
13. Number across: 1
14. Horizontal pitch (I did nothing, but it assigned 5.5”)
15. Number down: 12
16. Paper size: Custom
17. Page width: 5”
18. Page height: 11”

We've always used heavy painted metal shelf label holders but I wanted something more minimal and discreet. These were exactly what I was looking for. I was a little worried about the labels slipping out of the bottom of the holders (Why didn't they make these top loading??) but it hasn't been an issue at all. The plastic is rather flexible which I'm hoping means that they'll be less likely to crack.
Many people commented on the labels being glued onto the holder already. Now that I'm looking at them in person, I don't think the paper glued onto the holder is meant to be the label. I think it's to provide thickness and friction to keep the label you make firmly stuck in the holder. Without it, I can see how a paper label would be too thin to stay put.
Printing labels was easy. I just measured the sheet of labels that come with the holders, made an empty table in Word the same size with 12 cells, and printed it out on a regular 8.5" x 11 paper. This acted as my template. I went back to Word and filled in the text I wanted. I then taped the sheet of labels to my template and sent it through my printer.

I have used these shelf labels in the past. They are easy to place on the shelves and easy to change what goes in them.

Easy to Use!
These have been great to update the shelves in the library. I created a table in word that had the same size (3.74" x length) and added small clipart so students could navigate the sections of the nonfiction a little easier. Very easy to work with and the bottom loading isn't an issue. Everything stays in place and is easy to put back if students push them out looking through the book.

Easy and Quick Shelf Labeling
I used a label maker to quickly make shelf labels and stick them to the inserts provided. Had all my shelves labeled in two days! The kids love being able to easily locate books now. Any time I need to change the label I can just make a new label and stick on top of the old one.

We are establishing a small library and have hundreds of books yet to catalog and
